Abstract(s)
O presente relatório de estágio clínico surgiu no âmbito do 1º Curso de Mestrado de Enfermagem Médico-
cirúrgica na área de Enfermagem à Pessoa em Situação Crítica.
Este está organizado em duas partes. A primeira parte, tem como objetivo o aprimoramento das
competências gerais e específicas relacionadas com a Enfermagem Médico-Cirúrgica na área da de pessoa
em situação crítica. Na segunda parte expõe-se um trabalho de investigação desenvolvido no âmbito de
comunicação de más notícias à pessoa em situação crítica/família, com o objetivo de conhecer a perceção
dos enfermeiros e médicos quanto ao processo de comunicação de más notícias à pessoa em situação crítica
e sua família, em contexto de Urgência/Medicina Intensiva. A comunicação de más notícias é um
componente essencial da prática clínica que pode causar um impacto significativo no bem-estar emocional
dos doentes e suas famílias, e em ambientes de serviços de urgência e cuidados intensivos, onde as decisões
carecem de ser tomadas rapidamente, a comunicação de más notícias pode ser especialmente difícil.
Foi realizado um estudo descritivo e transversal de cariz quantitativo, com a aplicação de um questionário
a médicos e enfermeiros que exercem funções nos serviços de urgência e cuidados intensivos, de uma
Unidade Local de Saúde da região Norte de Portugal. A amostra foi composta por 67 profissionais que
concordaram em participar no estudo de forma voluntária, utilizando um método de amostragem não
probabilística por conveniência, em que a maioria da amostra pertence ao sexo feminino, com uma idade
média de 42,28 anos e maioritariamente constituído por casados. Metade da amostra relatou ter recebido
formação específica durante sua formação académica. Lidar com as emoções do doente e ser honesto sem
o privar da esperança, são as principais dificuldades. Em relação às estratégias de transmissão de más
notícias, com maior frequência média procura usar uma linguagem clara e acessível, de forma a evitar o
uso de termos técnicos, para que o doente entenda as más notícias. Além disso, há uma disponibilidade para
dar resposta a todas as perguntas e clarificar incertezas, permitindo que o doente ou familiar tenha tempo
para processar todas as informações transmitidas.
O estágio clínico permitiu o desenvolvimento de competências para a prestação de cuidados à pessoa em
situação critica. As competências da transmissão de más notícias são uma componente fundamental na
prestação do cuidar aos doentes em situações críticas. Durante este percurso, houve a possibilidade de
cuidar deste doente e familiares, durante o processo de doença, com foco na oferta de cuidados de uma
enfermagem especializada, centralizados nas carências do doente crítico e da sua família.
This internship teaching report was produced within the scope of the 1st Master’s Course in Medical-Surgical Nursing in the area of Nursing to the Person in Critical Situation. This is organized in two parts. The first part aims to improve general and specific skills related to Medical-Surgical Nursing in the area of critical person. In the second part, a research work developed within the communication of bad news to the person in critical situation/ family is presented, in order to know the perception of nurses and doctors about the process of communicating bad news to the person in critical situation and his family, in the context of Emergency/ Intensive Care. The communication of bad news is an essential component of internship practice that can have a significant impact on the emotional well-being of patients and their families and in emergency and intensive care settings where decisions need to be made quickly, communicating bad news can be especially difficult. A descriptive and cross-sectional study of quantitative nature was carried out, with the application of a questionnaire to doctors and nurses who perform functions in emergency services and intensive care of a Local Health Unit in the North of Portugal. The sample was composed of 67 professionals who agreed to participate in the study voluntarily, using a non- probabilistic sampling method for convenience, where the majority of the sample belongs to the female sex, with an average age of 42, 28 years and mostly made up of married. Half of the sample reported having received specific training during their academic training. Dealing with the emotions of the patient and being honest without depriving him of hope are the main difficulties. Regarding the strategies of transmission of bad news, with a higher average frequency seeks to use a clear and accessible language, in order to avoid the use of technical terms, so that the patient understands the bad news. In addition, there is a willingness to answer all questions and clarify uncertainties, allowing the patient or the family member time to process all the transmitted information. Internship teaching allowed the development of skills for the provision of care to the person in critical situation. The skills of transmitting bad news are a fundamental component in providing care to patients in critical situations. During this course, there was the possibility of caring for this patient and family members, during the disease process, focusing on the provision of care from a specialized nursing, centralized in the needs of the critical patient and his family.
