O transporte inter-hospitalar do doente crĂtico decorre da necessidade de transferĂȘncia de doentes entre instituiçÔes de saĂșde, por necessidade de cuidados especializados ou meios avançados.
Objetivo: Conhecer as vivĂȘncias dos enfermeiros de uma ilha de Cabo Verde no transporte inter-hospitalar do doente crĂtico.
Metodologia: Estudo qualitativo, no qual se recorreu a uma metodologia qualitativa, de anĂĄlise de conteĂșdo Ă s componentes discursivas, de uma entrevista semiestruturada, aplicada a uma amostra em grupo focal de 8 enfermeiros.
Resultados: Os enfermeiros associam ao conceito de doente crĂtico o doente em perigo iminente de vida e com necessidades de cuidados especializados ou meios avançados. E ao conceito de transporte de doentes associaram a transferĂȘncia de doentes entre unidades de saĂșde. Como sentimentos experimentados emergiram o medo, angĂșstia, cansaço, preocupação, indignação, impotĂȘncia, sufoco, tristeza, raiva, aflição, nervosismo, receio, stress, insegurança, responsabilidade, esperança, empatia, fĂ©, resiliĂȘncia, amor, força, otimismo, zelo, coragem, comprometimento, alegria, motivação, satisfação, confiança e alĂvio. Como dificuldades apontaram a carĂȘncia/dificuldades ao nĂvel dos meios de transporte; CarĂȘncia de recursos materiais apropriados para assistĂȘncia durante o transporte; Dificuldades para assegurar o conforto e a privacidade do doente; Problemas ao nĂvel da rede de referenciação/comunicação inter-hospitalar; AusĂȘncia do mĂ©dico durante o transporte; e custos relacionados com a impossibilidade de regresso do enfermeiro apĂłs a entrega do doente. Neste sentido foi questionado o que poderia ser melhorado no transporte, sugeriram a melhoria nas condiçÔes do transporte, implementação/atualização de formaçÔes em transporte de doentes e suporte bĂĄsico/avançado de vida e por fim a elaboração/implementação de protocolos na ilha. Relativamente Ă importĂąncia do enfermeiro no transporte inter-hospitalar emergiram: a elevada importĂąncia na estabilização do doente e a satisfação das necessidades multidimensionais do doente.
ConclusĂŁo: As vivĂȘncias da amostra no presente estudo mostrou-se que o transporte de doentes crĂticos traz muitas vivĂȘncias, cujo sentimentos, dificuldades vĂŁo de encontro aos relatados na literatura cientĂfica.
The inter-hospital transport of critically ill patients stems from the need to transfer patients between health institutions, due to the need for specialized care or advanced means. Objective: Knowing the day-to-day experiences of nurses on an island in Cape Verde in inter-hospital transport of critically ill patients. Methodology: Qualitative study, in which a semi-structured interview was applied to a focus group of eight nurses. Results: Nurses associate the concept of critically ill patient with the patient in imminent danger of life and the need for specialized care or advanced means. And the concept of patient transport is associated with the transfer of patients between health units. From the experienced feelings emerged fear, anguish, tiredness, worry, indignation, impotence, suffocation, sadness, anger, distress, nervousness, apprehension, stress, insecurity, responsibility, hope, empathy, faith, resilience, love, strength, optimism, zeal, courage, commitment, joy, motivation, satisfaction, confidence and relief. As difficulties, they pointed out the lack/difficulties in terms of means of transport, lack of appropriate material resources for assistance during transport, difficulties in ensuring the comfort and privacy of the patient, problems in terms of the referral network/inter-hospital communication, lack of the doctor during transport, and costs related to the nurse's impossibility of returning after delivering the patient. In this sense, it was questioned what could be improved in transport, and it was suggested that they improve transport conditions, implement/update training in patient transport and basic/advanced life support, and finally the elaboration/implementation of protocols in the island. With regard to the importance of nurses in inter-hospital transport, the following emerged: the high importance of stabilizing the patient and satisfying the multidimensional needs of the patient. Conclusion: The experiences of the sample in the present study showed that the transport of critically ill patients brings many experiences, whose feelings, difficulties go against those reported in the scientific literature. The improvement in transport conditions is the pillar to reduce the difficulties faced.
