- Aerobic exercise training in acute heart failure patients: an integrative reviewPublication . Delgado, Bruno; Oliveira, Isabel de Jesus; Gomes, Bárbara; Novo, AndréIntroduction: Decompensated Heart Failure patients are characterized by functional dependence and low exercise tolerance. Aerobic exercise can improve symptoms, promoting functional capacity and an increase of exercise tolerance. However, the benefits of early rehabilitation for inpatients have not been validated yet. Objective: To identify the use of aerobic exercise training in acute heart failure patients and, as secondary objective, to determine the feasibility, safety and effectiveness of such intervention. Methods: An integrative review with a search in PubMed, Cumulative Index to Nursing and Allied Health Literature, Scientific Electronic Library Online and Cochrane Library was conducted on January of 2020, to identify randomized controlled trials comparing patients who performed aerobic exercise training during inhospital stay, with patients who only performed usual care, published from January 2014 to December 2019. Additional hand search was performed in the national repository of academic works. The search retrieved 381 articles. Only one met the inclusion criteria. Results: One RCT was identified, with an assessment of high risk of bias, comparing patients who performed in-bed cycloergometer with patients who performed the same intervention with non-invasive ventilation and control group. The use of non-invasive ventilation lead patients to present a better performance, but even patients without non-invasive ventilation had significantly better results than the ones who only performed the usual care. Conclusions: Despite the results pointing some benefits, there is insufficient data to determine the feasibility, safety and effectiveness of aerobic exercise training in acute heart failure inpatients. More research is needed to validate its use.
- Cardiac rehabilitation to inpatient heart transplant—HRN4HTx intervention protocolPublication . Despite being successful, pre- and post-transplant limitations interfere with patients’ functional capacity, self-care, and quality of life post-surgery. Rehabilitation is necessary to address these limitations, prevent complications, and promote a safe return home. This study analyzes the safety of a phase 1 cardiac rehabilitation protocol (RN4HTx) in heart transplant patients and its effects on self-care capacity. A quantitative, descriptive study was conducted with 19 heart transplant recipients. The protocol was implemented in collaboration with a rehabilitation professional, who monitored adverse events, hemodynamic variables, self-care capacity (Barthel) pre- and post-transplant, and functional capacity at discharge (6 min walk test). The results showed that 68.42% of recipients were men, with an average age of 50.21 years and 15 days of hospitalization post-transplant. Approximately 73.68% of recipients were transferred from other wards with changes in functional capacity. All patients progressed to the final stage of the program without adverse events. There was a notable improvement in self-care capacity before and after transplantation, with a measure of functional status of 310.035 m (6MWT). The study found that RN4HTx is a feasible cardiac rehabilitation program without adverse events in the immediate postoperative period following heart transplantation, positively impacting functional recovery and therapeutic self-care capacity, thus increasing the safety of returning home. - Identifying elements for a cardiac rehabilitation program for caregivers: an international delphi consensusPublication . The analysis of cardiac rehabilitation (CR) components and caregivers’ needs suggests that some interventions may benefit them. Therefore, this study aimed to identify a consensus on the CR components targeting caregivers of patients with heart disease. Methods: A three-round international e-Delphi study with experts on CR was conducted. In round 1, experts provided an electronic level of agreement on a set of initial recommendations originating from a previous scoping review. In round 2, experts were asked to re-rate the same items after feedback and summary data were provided from round 1. In round 3, the same experts were asked to re-rate items that did not reach a consensus from round 2. Results: A total of 57 experts were contacted via e-mail to participate in the Delphi panel, and 43 participated. The final version presents seven recommendations for caregivers of patients with heart disease in CR programs. Conclusions: These recommendations are an overview of the evidence and represent a tool for professionals to adapt to their context in the different stages of CR, integrating the caregiver as a care focus and as support for their sick family members. By identifying the components/interventions, there is potential to benchmark the development of a cardiac rehabilitation strategy to be used and tested by the healthcare team for optimizing the health and role of these caregivers.
- Intervenções de enfermagem à pessoa com compromisso da deglutição pós-extubação: uma scoping reviewPublication . Oliveira, Isabel de Jesus; Novo, André; Azevedo, Paulo Manuel Dias da SilvaO compromisso da deglutição pós-extubação (CDPE) acarreta um conjunto de complicações que impactam negativamente a reabilitação destes doentes. No entanto, a evidência de intervenções para a abordagem terapêutica ao CDPE é limitada. Por conseguinte, identificar as intervenções relevantes neste contexto é imprescindível para a construção de programas de reabilitação baseados em evidência. Objetivo: Mapear as intervenções direcionadas à pessoa com CDPE. Métodos: Trata-se de uma scoping review, orientada pela metodologia proposta pelo Joanna Briggs Institute. Dois revisores pesquisaram a MEDLINE (via PubMed), a Biblioteca Cochrane, a Scielo, a Science Direct e a CINAHL em outubro de 2023. Foi efetuada uma pesquisa adicional de literatura cinzenta. A extração de dados foi guiada pelos critérios de inclusão: adultos submetidos a ventilação mecânica invasiva por mais de 48 horas e diagnóstico de compromisso da deglutição (população), CDPE (conceito) e unidades de cuidados intensivos (contexto). Os resultados são apresentados de acordo com o tipo de intervenção: estratégias compensatórias e reabilitação. Resultados: Identificados 1891 registos, acrescidos de 59 registos encontrados na literatura cinzenta. Após triagem e seleção, foram incluídos 14 artigos para análise. Os exercícios de fortalecimento muscular e os ajustamentos posturais são as estratégias de reabilitação e compensação mais frequentemente referenciadas. Conclusão: Estes resultados poderão contribuir para enformar o processo de tomada de decisão dos enfermeiros especialistas em enfermagem de reabilitação na construção de intervenções dirigidas à pessoa com CDPE. A relevância dada ao CDPE é recente, assim como a investigação desenvolvida neste domínio, o que foi corroborado por estes resultados.
